This is a scan of a NOS Payramid award card. Unfortunately it's for 1/2D, but you may be able to edit it.
My card in my machine is a bit tatty, but better than yours so I've left it. Hope this helps.

Just for comparison, here is another one. I saved the pictures from an Auction catalogue, (possibly Morphys?)

How could anyone replace the game with a pin table playfield? I know which one I prefer.

I'm an old fart, in my latter 60s, diabetic, mad as a coot, my body has bits that either drop off or joints that sound like a bag of old spanners being thrown around a drum factory by a baby elephant. I am what the authorities describe as in the high risk band . I think you may need to work on this...
